How do I become a host provider in Utah?
Our contract with the State of Utah requires that we take the following steps for each host home provider:
- Document candidate is at least 21 years of age.
- Run a criminal background check successfully (All others who are 18 and over and living in the home need to have passed a criminal background check.)
